Music Lesson Agreement

Thank you for registering for music lessons with NeuroMotif! Please read the following billing and scheduling information carefully before completing and signing the form.

Billing

Lessons operate on an ongoing, monthly basis. We will automatically charge your card on file on the 1st of the month for the upcoming month’s lessons (so, September lessons will be charged September 1st). We emphasize that students are billed for their time slot with their teacher, and are expected to attend at their scheduled time each week.

At the time of registration, the first month will be prorated as needed. Following that, you will be billed automatically on the first of each month while you are enrolled.

Unless written notice of discontinuation is provided before the billing date, students will be kept on the schedule indefinitely. If you would like to discontinue services, please let us know as early as possible before your last intended lesson, but no later than the last day of the month. We are happy to prorate your last month of lessons as long as you are able to notify us before the last day of the month, before billing.

If your card is declined when we attempt to bill, we will call you for an updated card number. We are not able to provide services until payment is received, so please provide updated payment information before your first lesson of the month. If your card expires or is cancelled for any reason, please get in touch to update it in order to avoid any billing issues.

Scheduling

Lessons will recur at the same time each week. The time slot students select at the time of registration will be kept on the schedule every month unless clients request to change times. Students can change their recurring time slot at any point depending on the therapist’s availability. However, time slots cannot be changed from week to week.

Any changes to the schedule must be handled by an adult over the age of 18.

Makeup Lessons

We ask you to do your best to attend your lesson weekly as scheduled, as this provides important consistency and maximizes your progress. We do understand that you may need to cancel occasionally, and will do our best to accommodate makeup sessions when possible. Makeup credits never expire, but makeups are not guaranteed as they are dependent on the teacher’s availability.

In order to be eligible for a makeup, please let us know you need to cancel your lesson by close of business the day before, usually 7pm (as always, the earlier you can notify us, the better). This allows the teacher to utilize your session time for another student. Day-of cancellations or no-shows are not eligible for makeups.

Students/parents are responsible for initiating makeup scheduling. Please check in with us when you cancel about what times are available.

If you miss your makeup, we will not be able to reschedule. In other words, we do not offer makeups for makeups.

If your teacher has to cancel a session for any reason, we will reach out to you with their availability for a makeup. If teachers are not able to make up a session they cancelled, we will credit that session to the next month’s invoice. Credits are not available for student-initiated cancellations.

  • I understand that continuation of services will be assumed unless I provide discontinuation notice in writing no later than the last day of the previous month.
  • I understand that my lesson will recur at the same time each week.
  • I understand that makeup lessons are not guaranteed.
  • I understand that in order to be eligible for a makeup lesson, I must provide notice by close of business the day before my scheduled lesson.
  • I understand that same-day cancellations and no-shows are not eligible for a makeup lesson.
  • I understand that I am responsible for initiating communication about scheduling a makeup.
